I don’t have the knee problems but do have some lower back issues. My Mustang Lowdown seat (with back rest) is pretty comfortable and works for me. As others have said, get some highway pegs so you can stretch your legs out. Take a few extra breaks during your ride, stretch out…

You can add 2" pull back riser for your bars and that will bring the bars closer. You probably wouldn't have to change any cables. You should really check out Mean City Cycles to have your seat modified with the memory foam as said before, this is not expensive and will work wonders for your back side. Good luck.

Also, you might want to try some abdominal exercises to strengthen the muscles that support your spine. That's the ONLY thing that worked for me. Now I can ride 8+ hours pain-free when before about 45 minutes was my limit. Back rests and all that other stuff is fine, but if you fix the underlying problem, then you can forget about the add-ons. At least that was my experience.

I personally don't think a handlebar change will help you with your backside or knees. You need a good quality seat. Do you have the same issues when you run the sundowner? A backrest may help take some pressure off your bass. not really understanding how riding can effect your knees, is it your leg position? maybe adding some floorboard extensions can help.
If you want apes, they can be done on the cheap. I changed mine out for $150. Paul Yaffe 10" Bagger apes. You can add these without having to buy any extended cables or wires. The bars run $90 at dennis kirk. You should already have the 1 piece riser clamp on you 2009, so that would be a savings to you. I changed my bushings while I was in there and had to get rid of my fairing mounted mirrors, and go back to the bar mounted ones.

Backrests are nice I've heard, I wouldn't know since I never had one. I was not aware that Apes cost a $1000 though. Everyone doesn't get as much comfort from apes as some. I gave up my OEM 10" mini apes for 10" straight up TBars and I feel much better riding now. The hand position is more squared off and just a bit lower than the apes I had. I used to get sore shoulders on long rides with the mini apes and I don't get that now.
If you have any friends with other bars try sitting on them. Everyone doesn't warm up to the same type bar. Those backrests are nice though, especially the ones I've seen that are on a hinge and fold down.

Ya 1000 bucks or more. I've talked to a couple of HD stealerships and they both tell me 8 hours of labor. 8 hours of labor at $95 a hour = 950 plus bars, cables, ect.
