I agree with the backrest suggestions. It makes backing my Ultra way easier.
I have a freind who rides a Fat Boy. She's so short, she had an airlift suspension installed. Flips the switch as she comes to a stop so she can reach the ground. Flips it again as she takes off and in a few seconds, it's back up to riding height. Probably cost prohibitive though.
Another (probably) cost prohibitive option no one mentioned is a reverse gear. I'm not suggesting it and won't. Just saying it's an option. You can read about one here.

The reverse gear pictured works well. I have a friend with a trike that has one. It's the same gear ratio as 1st gear if I am not mistaken.
