For my 01 RK I had the same issues, too wide and had to reach too far. The dealer had a set that were a little narrower, a little taller, and a little more pull back without having to change the cables. That worked out great. They were called Tall Boy bars that were 3.5" taller and 1.5 narrower.

OK, I got the Khrome Werks Heritage bars and not happy with them except for width (still leaning forward). I did however figure out what I am looking for as far as bars go.

The standard stock height is perfect. the width a little narrower and the way they angle to the side like the Heritage bars is good too. But they need to come back to me more like 3-4". Preferably 4"

I like to sit upright or a little leaning back with my arms slightly bent when I ride. At this point, I would be happy sitting upright with my arms stretched out. Not really though.

Know this is a tall order, but is there anybody that is 5'-8" like me and found a solution like I am looking for with their bars?

Maybe Harley's own Tall Boy?

Got the Tall Boy bars today and changed them out. What a difference! It's like a whole new bike. I could ride this setup all day without any discomfort.

Anyway, a few things. First was even though the bars were clamped as far as they could be clamped, if I pulled up or pushed down hard enough on the bars, they would move. Upon closer inspection, I noticed that the top clamps were touching the lower clamps and couln't be tightened any further. I removed the top clamps, one at a time, and and ground almost 1/8" off them. Re-tightened each clamp and they hold perfectly. I could whack the handlebars with a baseball bat and they wouldn't move.

Which brings me to my second issue. Once i buttoned everything back up, it looked like the bars were crooked. What I figured out was if I forcefully turned the bars as hard as I could left or right, the bars would go crooked in whatever direction I turned. I tightened the nuts for the lower clamps that are under the top triple tree and it's holding much better but can still go crooked if i push/turn hard enough.

Is there some kind of shim/spacer that I need to hold the lower handlebar clamps from being able to move? Are the rubber mounts in need of replacement? Where do I go from Here?

Yes replace the rubber mounts also maybe go with a 1 piece mount. I did both and it helped. They are still a tad off, but much better.

Mine did the same thing so I just made sure everything is adjusted right and left them be. I pulled on mine to get them even and the have stayed that way. I have to turn hard against full lock to get them out of alignment.

Default Stock bars

I have a 2012 Road King. Changed to a 2014 Road King stock bars. Big difference. Should have done it years ago.
