I'm not convinced so much that the bolt is the problem, regardless of which torque spec is used, or the quality if the levers and/or shafts. When I look at the levers, there is a lot of meat there that needs to be pinched tight around the shaft. And given the dia if the bolt, it'll be hard to pinch the levers tight without taking the bolt to its limits. Seems a better fix is the levers need to be made with either a little less meat to them, or a relief slot added to the opposite side of the shaft from the bolt hole, allowing the bolt to pinch the levers tighter by having less material to overcome in order to pinch.

It pinches down perfectly as designed, as long as it is kept tight. And if too much metal was removed you would loose structural rigidity, and or the spring effect would be lost, and the metal would not open up when the bolt was removed, making removal more challenging.
