Remove the offending shift lever and simply shorten the existing splined shaft. Easy-peasy with either a hack saw or a cut-off wheel on a die grinder.
File the sharp edge over and reinstall the toe shifter and you'll have no need for any ugly spacers or caps (that catch your boot). You'll also have a bit more foot room.
Takes less than an hour and a replacement splined shaft costs around 15 bucks should you ever want the heel shifter back.

A lot of good posts with solutions before mine. But here's how a cheap redneck does it.
My "spacer" is the rubber part of the old heel shifter cut down a little and placed inboard of the single shifter I moved out. It works well and didn't cost me anything.
