This ball definitely got loose on him. The hole is now oblong. Told him to buy a new receiver before this one really causes major issues. Doubt he will

Yup, lock washer was on. Just after awhile they need to be re-torqued . A lot of force moving that ball back and forth. Especially on our shitty roads.
