The key was totally missing, the yoke had one set screw in it. It must have at one point been screwed in far enough to go into the key way?

Sounds like they were using the set screw as the drive instead of a key.

Apply liberal amounts of antisieze to the shaft! Once the yoke is on use a center punch in the Allen holes. Gives the Allen's more to bite on.
