Default Clutch cable keeps getting loose..

I have a 95 Annv. 1200 C.

My issue is I keep having to adjust my clutch cable.. I've hand to adjust it 2 time in the last week.. the clutch seems to be acting fine.. but the cable keeps getting too much slack in it and when I have the clutch all the way in I feel the bike wanting to creep on me.. I know your suppose to have about 1/16 - 1/8 inch gap between the lever and the cable.. but as of right now it has about 1/3 of an inch gap.. I checked the adjustment screws and nothing seems to have moved..

any suggestions on what could be causing this?
