Pull it out full ,start up drive up my driveway, close door put at the half position drive 3 city blocks push all the way in. This has worked great since 2001 on my 883.

Very sparingly... I use it to get the bike started (cold weather) push it in immediately and play with the throttle a little set the throttle drag just a few hundred rpms above idle until it warms up. In the summer I don't use it at all. A couple of pumps on the throttle, start and let it idle or just above idle for a minute or two.

I've been told by a couple of techs that using it a lot will cause build up on the plugs. I just changed my plugs (10K on the bike) and the plugs still looked pretty dang good.

I know what the book says... so maybe they know best... then again... I tend to go with the people who actually work on the bikes...
