My Clutch Is Maladjusted. Did I Raise It Wrong?

well i'm glad i ran into this thread.
I had this problem the past couple of weeks, clutch kept getting out of adjustment (only fully engaging, just at the end of the level travel) But I had not adjusted behind the derby cover. I finally did that, and it was 1-2 revolutions loose. So I tightened that up with 3/4 turn back and was good for about 2 weeks, and then it got loose again.
Checked the derby cover, but it wasn't that loose, so I tightened it and then used 1/4 turn back and now it's been good for a couple weeks.
Is a new clutch pack / spring plate in my future or did I just not do it right the first time?
