If U have shorter arms U might also want to look into a seat that will push U forward an inch or so that have a built in back rest. Several brands make them.
I didn't ride several yrs due to a broken back and it took a couple weeks to get my back muscles in shape for a no pain ride.., but after about 1k miles or so I could ride pain free with stock seat and bars for 3-5 hrs.

My sig pic is before my mod, but I had the same problem as you. I put on Wild One Chubby 12's apes, turned them down just a touch. They are still basically in line with the forks. They are wider, which brought my shoulders back and the pain was gone. Now I can ride all day.
In addition, I bought a driver's backrest, which works good also. Mine only works with my bike set up as solo though.

I had a rotator cuff surgery as well so I definitely needed the pullback as I believe you do. The Wild 1 Apes have a lot of pullback (12") vs stock Heritage bars so definitely consider them. I have an 07 so my bars were not the mini apes that come on the new bikes. Try Wild 1. Made all the difference in the world to me!
