I need a sportster clutch guru...

I'm having issues keeping the wifeys sporty clutch in adjustment. I put in an ez clutch last fall. all is well until I decided to re-adjust it last week. Now it falls out of adjustment all the time.
Question(s): Which way does the locking nut that goes over the adjustmet rod go, flat side out or ribbed or ridged side out?
Is there any kind of grommet or rubber gasket that sets inside the derby cover that holds it in place (not talking about the gasket that seals the cover itself). on the inside of a sporty derby cover there is a ring moulded in the cover that looks like it would hold the locking nut in place. is there something that goes inside this ring that holds the locking nut in place?
what am I missing here to keep this clutch in adjustment? It is as if I adjust it, she rides it and it vibrates out of adjustment and leaves her without a clutch at inopportune times.
