Default Shift lever woes

I've got an 09 RK, and since buying it new, I've had to replace the rear half of the heel-toe shift levers at least four times. Is this normal? I had one simply disappear while on the highway once, and its replacement held on for awhile, but then came loose twice or three times, no matter how much I tightened the bolt or applied locktite.
Eventually I took the rear shift lever off my wife's bike (she's a dedicated toe shifter), and that held fine for awhile, but this morning it fell off! I saved it, but am growing increasingly frustrated.
Is there an insider trick to keeping this things on?

A lot of times the bolt will bottom out before the pinch is tight, try putting a small washer under the bolt head and then torque it down tight. Also check the splines on the rod to make sure they are not shot, if so replace it and use the above method. We have actually put a small tack weld on them before for people traveling through when we didn't have replacement parts on hand. The good thing is it will not come off on its own from here forward; the bad, you have to grind off the weld to remove it if necessary....

Thanks, I'll try the washer - on my last one I had that problem, that the bolt bottomed out before the pinch was tight, so maybe that will fix it!
