What lane do you ride in?

I know the one main thing I always assume everyone is trying to kill me while riding. My friends ride like they just assume a car is stopping at the intersection because the light is red, or they just assume the car will turn because they have their directional on. I drive 50k plus a year for work in a car and see a lot of stupid assumptions like that, that end up in accidents.
The one thing I never do on my bike is assume anything but the fact that they are trying to kill me at any chance they get. I get called slow poke and picked on for smelling the roses, cars beep their horns at me for not pulling out in front of people while leaving parking lots. But you know what at the end of the day I am still alive and still riding. Complacency kills, as long as you don't ever forget that you will be in a much safer environment.

I'm almost always in the middle lane, sometimes the left lane if middle lane traffic isn't going fast enough. I avoid right lane because they're always doing ~10 mph under the limit and constantly having to speed up and slow down for on-ramps & off-ramps.

Depends on traffic.
I think your ideas on riding in the right lane are a bit off, especially on surface streets. You are likely harder to see there and you put yourself nearer to more potential dangers.
