I didn't use any and it basically made it into solid risers.
Using a smaller diameter spacer or washer under the extensions would allow the bushing the free play needed to operate normally. It just wouldn't look as clean.
I haven't noticed enough vibration to have me looking for a cure setup as I have them.

Alright, alright, alright...got 'em on and love them. I think they look cool, but more importantly, they brought my grips back up where they used to be with my apes. I didn't use a spacer either and found that there was still plenty of bushing "squish" as I torqued them down. They are flush with the tree, but I think the bushings are still doing their thing.
Thanks Kowan for the reply that pushed me forward with these. The quality seems much better than the LA Choppers extensions I looked at.
It's a shame Novello has discontinued these, but I got a great deal on them because of it on ebay from Wilsoncycles.
If anyone is looking for a set, I recommend getting them while you still can.
