I've seen bikes fall off jacks before, but never due to lift failure...they depended upon the bottle jack to hold, and found the bike on the floor in the morning as the lift slowly lowered. Blocking the damn thing up makes all the difference.
I do most of my rear end work with the triple tree secured via staps into big eyelets threaded into floor inserts, and a thin scissors lift under the frame or engine. Even with that set-up I block the mid-point of the bike up if it's going to be suspended for any length of time. It's also easy to pull a front wheel by placing the same rig under the frame, leaving the rear wheel in gear, and putting the kickstand on a board.

Do you mean that there is something better on the market than what I've been using???? I've just taught momma how to swivel the bike a bit so I can get the stands just right!!

Just didn't fit under the bike right. When lifted, I couldn't get it to hit 4 good spots on the frame. Didn't like that way it sat on the lift in the best possible position.
