Another vote for left foot down, right on brake, and in first ready to go.
The only time the right sees the deck is when I'm on some real screwed up asphalt, or it's time to take a break and get off.
I refuse to get bitched by a cager.

both down and up at the same time for me, leaving it in first at all times, unless you know it's going to be a long wait, then maybe toe it into neutral. just don't forget being in neutral, it's never good to take off in neutral, especially with that impatient cager right behind you

Same here. I tried just putting my left foot down, but found it more natural with both feet firmly planted. I can get both feet up just as fast as one when taking off.

I suggest you guys that do, quit putting the bike in neutral
It is not safe in the event you need to move quickly
Also everyone should learn to take off with only the left foot on the ground and the right foot on the brake. This method is the safest way to take off from a dead stop heading up hill
