I would think anyone opening any bike related business should have enough knowledge of their clientele base that you wouldn't need to rely on a computer survey of strangers who may or may not be telling the truth.

I stopped at the DOB thing which is not needed at all. Age would have accomplished the needed info.
A specific name and dob resembles "fishing" to me.

Chris, it might be better if you started by telling us about your business in more detail before you start asking questions about DOB, etc.
Seems you could get a ton of this info just by visiting your local FL bars.
Kind of funny you asking if we own a bike. I'm pretty sure everyone on a Harley motorcycle forum owns one.

Any of you give me that much information and you will be shocked what I will tell you about you and your closest relatives.
If he has Lexis/Nexis access, you are giving him enough to go very deep into your personal information.
OP, Give us your name and date of birth and city and state.
While you are at it, give us the location of this bar. Unless it is a new facility, someone on here has likely had a beer there under its previous name.

Just a suggestion, use year of birth for demographics & age verification. Sure, you will exclude some possible people (under 18 or 21 I assume), but ill give you my year of birth, but not my birthdate. Of couse I could make one up. . . . but that's not my style either.

So you did not ask the question . What are some important things a good bikers bar should have ? And if you did my answer would have been .
A tool box , a board for posting rally's , bike sales , and tattoo shops . Front parking for bikes only . A dry front porch for smoking on a rainy day. And maybe a dart board or two .

I understand why people are reluctant (and wise) not to give out their birthdays, but what you can do is just pick 1-1 of either your birth year or an adjacent one. That is close enough for the demographic, without revealing any PII. I do that, and sometimes forget it, then on new year's, I get a bunch of happy birthday messages from web sites.
