Oil Filler Cap Blew Off??

Oil Filler Cap Blew Off??
Out on my '99 ETG Standard yesterday, rounded a corner, cracked the throttle and POOF the oil filler cap blew right out! Oil everywhere.
One of the guys said he'd seen this once before but nobody else had. Any ideas as to what would cause this? Yes it was in tight. Could it be an age thing since the bike is 11 years old?
