forwards and stirrups question

Can't believe I'm asking this....blast me if you want. Am I suppose to set these up flat so my whole foot goes flat on them or do I raise the heal part up so that my foot slides back into it and not onto it. I can't seem to find a good way to position them. They look like this.

OK, basically you use it while cruising, after all the shiftings done. So your foots at an angle with heal on the heal rest.

Looking at them again. I feel like my stirrup is too close to the peg to wedge my heal on and to far out to not have it on all the time. Does this look wrong? They aren't comfy after a few minutes my feet want to slide backwards right now there just seems to be no right way. I'm doing something wrong

Here's what I did, cut up an old pair of spurs and welded a strap along one side for mounting. Feet stay in place even when getting a blast of wind from a semi on a 2 lane.

I use em on my FXSTC, Im tall and need forewards. The left side I positioned so that with my heel on it I can just flip the shifter up with my toe. The right side is a little lower so I can slide my foot onto the brake easier. It's whatever works for you.. They made my bike totally comfortable for me...Tighten that big nut real tight, they like to come loose,,
