It may be a discontinued aftermarket fender ... I'd have a look at Custom Chrome, Tucker Rocky, Drag Book at the fender sections ... not guaranteeing you'll find it ... but, if anywhere, probably one of those sources ...

Since FXDX were discontinued in '05, whoever made it may have discontinued with a fall off in demand ...

Another option might be to get a fender blank and cut the shape out ... I believe a fender blank should be pretty easy to come by and you could fashion it into that into one like in the photo ...
