The top hole (left on the tracing, narrow end) is where you put your peg. The bottom hole (what I think you're calling 'middle') is where you bolt it to the frame. The two smaller holes that don't go all the way thru are where the stud goes that lets them be adjustable.

If you look at your frame you'll see two threaded holes. One is for the bracket to mount, the other takes a short piece of all-thread that has an allen head socket in the outer end. Which hole in the bracket you put over that all-thread determines how far back/forward the peg ends up.

H-D used to have the instructions for these on their website - check it out. Those pdf files can be extremely useful although I've noticed HD isn't posting as many as they used to.

No problem. I'll take it to work and scan it tomorrow. I'll email tomorrow evening - can't access personal email from work.

I have the standard non-adjustable sort on mine if you need a look at a different style.

Just a thought... I picked mine up on ebay for 30$ shipped complete with pegs and mounts. The standard set (these) are factory painted black.. You can see them in my (badly outdated) sig pic
