I left my stock mid pegs on when I put on my forwards. I pulled the rubber off and cut about an inch off the length, so they don't get in the way.

This is essentially what I do. Except my feet are big enough to reach between the pegs themselvs without the heelrests
If you've got a size 11 boot or larger you could do the same.

I've also got the mid pegs on my 08 Fat Bob, and they're great to flip down on the highway. Cost me more than I expected for the parts, but that's HD right?

I used both points to make my own floorboards.

I rode two hours at 80 mph today, and the mids were majorly comfy! A lot less fatigue with them. I let my toes rest on the rear of the boards.

I added the mids to my Fatbob and have to say, my favorite mod so far. You get the 2 position for long rides, but also get to stand up to get some blood flowing again for the really long rides.

Glad I asked. Sounds like a must-do. I guess I better add it to my never-ending mod budget and start the search for a low cost set of mid mounts.

Yep. I have a fused left ankle and a rebuilt right knee from an accident 1.5yrs ago. So I shift with my left heal (Achilles area), and brake with my righ heal (Bottom). I kept my mids in place, and use a Linby Bar for adjustable highway pegs. I ride 95% on the highway pegs with my feet in front of my controls and use the mid pegs to loosen up my knees on long rides. Best of all worlds.
