Here's one example I found with a quick search, asking $50 plus shipping. If you do that, you can sell your forwards and pocket some $$$! Good luck!

dacfan, it is very easy to go back. Only thing that creates a problem is if the mid shifter shaft has been removed. If so then you will have to remove the inner and outter primary. If when they put the forwards on they left the mid shaft in then you can change the forwards back to mids in less than an hour with just cpl wrench's.
To find out if your shifter shaft is still there just look at your outter primary in the center see if there is a cap there that is flush or sticks out if it sticks out its still there if its flush just double check and look back on the back side of the primary and see if there is and extra lever it will be laying ther or it will be hooked to the back lower jug at the bolt.
If that lever is still there then the switch is a simple if its not then you will have to remove the entire primary to put it back in.

also as to price you can find mid controls all over the net and if you look hard enough you will even find someone that is willing to swap you even up. I just did this with guy that is now a member on this site found him on craigslist i took my uncles mids off his bike and drove to the guys house removed his forwards and put my uncles on his bike then went home and put the forwards on my uncles bike. Went quick and easy but his shifter shaft was still in his primary.
