they put new rear tire on and the plate that keeps the bolts for backing out and I requested brand new bolts.

Probably was loose for a while with the steel bolts beating out the aluminum tapped holes, Since it went so far that it finally sheared of the bolt, my guess is the cog pulley mounting flange is torn up to crap.
But of course just an educated opinion from seeing a few on this Forum.
The FP doesn't want it to happen again. Getting new bolt to stay tight in those holes would be shear luck.
Keep in mind, the FP needs the help. So direct your questions or help to him.
Did the bolt heads look new? Why did you request the locking plate? Had this happen before?
I assume it was a big ring with 5 holes and a tab at each hole to bend up and act as a lock for the head?
I can see them getting loose but not backing out. Just how bad are you beating that old girl?
There's the bolts, let's see locking plate and wheel hub and a close up of the tapped holes.
There's your problem I believe, no sign of dry coat locking compound. See any white (dried red) loctite or evidence in threads of hole? Maybe grade marking got knocked off but I see no grade marking. See any evidence of 6 dashes on the head?
Need a picture of end of head.

dont know yet it was towed to a Harley dealer obviously not the one who installed everything. Service for a QUICK once over only saw the pulley was messed up as of right now. just trying to pick everyone’s brains on a solution to this problem that I see this is very common after looking it up
